One (1) free medical checkup voucher (“Voucher”) worth S$25 will be given to each successful new application of or renewal of existing United Foreign Domestic Worker Insurance policy (“UFDWI”). |

In the event that the UFDWI is cancelled and the same applicant/ employer applies for another UFDWI on behalf of another new foreign domestic worker within six (6) months of the first UFDWI policy inception date, such Voucher issued in respect of the first UFDWI may be transferred to the new UFDWI. |

| 3. |
The Voucher is redeemable at any Singapore Raffles Medical Clinics, apart from the Raffles Executive Medical Centre. |

| 4. |
The Vouchers are given while stocks last. |
| |

| 5. |
The Voucher is not transferrable or exchangeable for cash or other payments in kind or used in respect of any other medical treatment. UOI is not obliged to replace the Voucher if it is defaced, torn, lost or stolen. . |

Other terms and conditions relating to the UFDWI, the Voucher and the medical checkup imposed by UOI and/or Raffles Medical Group (as applicable) would also apply. |

UOI reserves the right to change the terms and conditions of this promotion or terminate or suspend this promotion without prior notice. |
